Researchers alarmed at rise in hookah use among California youth

ScienceDaily (Aug. 19, 2011) ? Hookah use among California youth ages 18 to 24 is rising rapidly according to a study conducted by researchers at University of California, San Diego School of Medicine.

The study appears in the "First Look" online version of American Journal of Public Health.

Researchers say the increased popularity of the hookah -- a water pipe used for smoking tobacco -- may be caused by the social nature of the behavior coupled with the misguided belief that it is less harmful than cigarettes.

"This rise is particularly alarming because it's happening in California, a state that leads the nation in tobacco control," said Wael Al-Delaimy, MD, PhD, associate professor and chief of the Division of Global Health in the UCSD Department of Family and Preventive Medicine. "While cigarette smoking has decreased nationwide and in California, reports of ever using hookah have increased, especially among adolescent and young adults."

Most users report smoking hookah with many friends, according to Al-Delaimy. "Though public indoor cigarette smoking is banned throughout California, hookah use is permitted in designated lounges. This may create the impression that hookah is a safer alternative to cigarettes, which is simply not true."

The UCSD research team used data from the state-wide California Tobacco Surveys that are led and directed by Al-Delaimy. This data on hookah is the largest representative sample on hookah use collected at different points in time from the same source population.

Study results showed that from 2005 to 2008, hookah use among all adults increased by more than 40 percent; and by 2008, hookah use in California was much higher among young adults -- 24.5 percent among men, 10 percent among women -- than it was among all adults -- 11.2 percent among men, 2.8 percent among women.

Al-Delaimy's team also found that hookah smoking among men and women was more common among non-Hispanic Whites, with at least some college education. Unlike cigarette smoking -- where those with higher education smoke less -- hookah use is higher among those who are more educated.

"More specific studies are warranted but we urge policymakers to consider laws that would ban hookah lounges, thus eliminating the implication that hookah smoking is safer and more socially acceptable than cigarette smoking," said Al-Delaimy.

Will Miami case lead to 'fundamental change'? (AP)

CORAL GABLES, Fla. ? The latest scandalous allegations in college football ? this time at the University of Miami ? have renewed talk by the NCAA of the need for "fundamental change" in athletics.

And Hurricanes officials say they're eager to resolve the case.

Former booster Nevin Shapiro, now serving 20 years in federal prison for his role in a Ponzi scheme, claims he provided Miami players with prostitutes, cars and other gifts over the past decade.

"If the assertions are true, the alleged conduct at the University of Miami is an illustration of the need for serious and fundamental change in many critical aspects of college sports," NCAA president Mark Emmert said in a statement.

In the past 18 months, football teams at Southern California, Ohio State, Auburn, Oregon, Michigan, North Carolina, Georgia Tech and LSU have been investigated or sanctioned by the NCAA.

Last week, Emmert led a group of university presidents ? including Miami's Donna Shalala ? in drafting an outline for change in college sports.

Shalala said she was upset, disheartened and saddened by Shapiro's allegations.

"We will vigorously pursue the truth, wherever that path may lead, and I have insisted upon complete, honest and transparent cooperation with the NCAA from our staff and students," Shalala said in a statement.

Most cases are resolved in six to seven months, but more complex investigations take longer, an NCAA official said.

Shapiro began making his allegations about a year ago. He told Yahoo Sports that 72 football players and other athletes at Miami received improper benefits from him in the past decade.

Shapiro was sentenced to prison in June for masterminding a $930 million Ponzi scheme, plus ordered to pay more than $82 million in restitution to investors. NCAA investigators were on the Miami campus this week and have interviewed Shalala and athletic director Shawn Eichorst.

Eichorst was hired as athletic director in April to replace Kirby Hocutt, who became AD at Texas Tech. Al Golden became the Hurricanes' coach in December after Randy Shannon was fired.

Golden's entire team practiced Wednesday, even though the claims by Shapiro involve several current players. Golden said it was too soon to take disciplinary action. His team opens the season Sept. 5 against Maryland.

Yahoo Sports published its story Tuesday, saying it spent 100 hours interviewing Shapiro over the span of 11 months and audited thousands of pages of financial and business records to examine his claims, some involving events nearly a decade ago. The NCAA's four-year statute of limitations doesn't apply when there is a pattern of willful violations that continues into the past four years.

A person familiar with the situation said much of Shapiro's access to Hurricane programs in recent years was approved by Hocutt. The person spoke to The Associated Press on condition of anonymity because of the ongoing investigation.

Hocutt, the person said, allowed Shapiro on the sideline before football games at times during the 2008 season, plus invited him to select gatherings reserved for the athletic department's biggest donors.

"That's what Kirby did," the person said. "His No. 1 job was to raise money and this Nevin Shapiro guy was one of the few people Kirby could get to write checks."

In a statement, Hocutt said Shapiro was treated like other members of the Hurricane Club.

"While I was athletics director, the benefits and experiences Mr. Shapiro received were consistent with those provided to others at his membership level," Hocutt said. "I never personally approved any special access for Mr. Shapiro to university athletics events or programs."

Larry Coker, who coached the Hurricanes in 2001-06, said he had not been contacted by the NCAA or Miami about the investigation. Any coach or athletic direction involved in the case who now works at another school could be subject to NCAA punishment if found guilty of a violation.

The AP interviewed more than a dozen former Miami players, and their reactions ranged from denials of involvement to declining comment.

Vitamin D Levels Linked to Certain Skin Cancers, Study Finds (HealthDay)

MONDAY, Aug. 15 (HealthDay News) -- The higher a person's vitamin D levels, the higher the risk of non-melanoma skin cancer, such as basal cell carcinoma and squamous cell carcinoma, finds new research.

But the study, appearing in the Aug. 15 issue of the Archives of Dermatology, stops short of saying that high vitamin D levels might actually cause these types of cancer, the most common malignancies in the United States.

And because ultraviolet (UV) radiation exposure is necessary for vitamin D production in the body, it might simply mean that people with more sun exposure tend to develop more non-melanoma skin cancers. It's unclear whether it's the damage from UV rays that accounts for the risk, or rising vitamin D levels that accompany exposure to the rays.

"This adds to the murky water [surrounding the relationship between vitamin D and skin cancer]," said Dr. Vijay Trisal, assistant professor of surgical oncology at City of Hope Cancer Center in Duarte, Calif. "Is it vitamin D or sun exposure? The two go hand-in-hand."

Other scientists have investigated a possible relationship between vitamin D and skin cancer, but so far the results have been limited and conflicting.

One study suggested that higher vitamin D levels might actually protect against skin cancer. This could be because vitamin D may inhibit a pathway involved in cancer, said Dr. Melody Eide, a dermatologist with Henry Ford Hospital in Detroit, and lead author of the current study.

But two other studies had results suggesting the opposite.

Eide and colleagues based their findings on 3,223 mostly female, white patients in a Detroit health maintenance organization who had visited a doctor either because they had osteoporosis or low bone density.

Many more patients (2,257) had too-low levels of vitamin D than had adequate levels (966).

Over a follow-up period of almost 10 years, 163 participants developed basal cell carcinoma, 49 developed squamous cell carcinoma, and 28 developed both.

Those with vitamin D levels above a certain threshold had a 70 percent greater risk of developing one of these cancers. (That threshold was 15 nanograms per milliliter; people with less than that were considered deficient in vitamin D.)

People with higher vitamin D levels also tended to develop their skin cancer on parts of the body not typically exposed to sunlight, like the arms and legs, but that finding was not statistically significant, the researchers reported.

At this research stage, it's difficult to untangle the possible mechanisms behind this.

"It's a triangular relationship between UV light with the production of Vitamin D and the induction of skin cancer," Eide said. "That makes it difficult to know."

The study didn't take into account lifetime sun exposure, family history of skin cancer, vitamin D supplementation, exercise, smoking or several other factors that might have influenced the outcome of the study. In addition, the study authors noted that it was "highly likely" that the participants' exposure to sunlight might have skewed the results.

"We need some measure of lifetime cumulative UV exposure, which is very difficult to measure," Eide said. "We tend to move around a lot; people go on vacations. There could be critical windows during our life."

Wisconsin GOP's stand could reverberate elsewhere (AP)

MADISON, Wis. ? A stand by Wisconsin Republicans against a massive effort to oust them from power could reverberate across the country as the battle over union rights and the conservative revolution heads toward the 2012 presidential race.

Democrats succeeded in taking two Wisconsin state Senate seats away from Republican incumbents on Tuesday but fell one short of what they needed to seize majority control of the chamber.

Republicans saw it as a big win for Gov. Scott Walker and a confirmation of his conservative agenda, the hallmark of which was a polarizing proposal taking away most collective bargaining rights from public workers.

"Republicans are going to continue doing what we promised the people of Wisconsin ? improve the economy and get Wisconsin moving back in the right direction," Republican Senate Majority Leader Scott Fitzgerald said in a prepared statement after the victory.

Walker attempted to strike a bipartisan tone in victory, saying that he reached out to leaders in both parties.

"In the days ahead I look forward to working with legislators of all parties to grow jobs for Wisconsin and move our state forward," Walker said in a prepared statement.

Democrats and union leaders tried to make the best of the historic GOP wins. There had been only 13 other successful recalls of state-level office holders nationwide since 1913.

"The fact of the matter remains that, fighting on Republican turf, we have begun the work of stopping the Scott Walker agenda," said Democratic Party Chairman Mike Tate.

Phil Neuenfeldt, president of the Wisconsin State AFL-CIO, said voters sent a message that there is a growing movement to reclaim the middle class.

"Let's be clear, anyway you slice it, this is an unprecedented victory," he said.

Still, it was far less than what Democrats set out to achieve. And while they still plan to move ahead with recalling Walker, maintaining momentum for that effort which can't start until November will be difficult.

Sen. Luther Olsen, one of the four Republicans who won, said he hoped the victories would "take the wind out of the recall for Walker, but I'm not sure."

Two Democratic senators face recall elections next week, but even if they prevail, Republicans would still hold a narrow 17-16 majority.

Four Republican senators held on to their seats Tuesday. They were Olsen and Sens. Sheila Harsdorf of River Falls, Rob Cowles of Allouez, and Alberta Darling of River Hills. Two Republicans ? Randy Hopper of Fond du Lac and Dan Kapanke of La Crosse ? were defeated. Former deputy mayor of Oshkosh Jessica King beat Hopper and Democratic state Rep. Jennifer Shilling beat Kapanke.

A ninth senator, Democrat Dave Hansen of Green Bay, won his recall election last month.

Collectively, more than $31 million has been spent on the recalls, largely from outside conservative groups, unions and others.

Republican and Democratic strategists were leery of reading too much into the results heading into next year's campaign in which Wisconsin is expected to be a key swing state.

Democratic strategist Chris Lehane said the results could provide "some early radar warnings" about the 2012 races.

"At a minimum, we already know that the conservatives are providing energy for progressive to fight back like an angry badger that otherwise may not have existed," he said.

Lehane said Wisconsin's tumultuous year since November's elections has been a microcosm of the current "rollercoaster" era of U.S. politics.

Wisconsin voters had mixed emotions about the necessity of the recalls.

Wayne Boland, 41, a Whitefish Bay man who works in marketing for a medical equipment maker, said he voted for the Republican Darling "not because I entirely agree with everything the Republican Party has done or the governor" but because they're working toward addressing the state's problems.

Republicans won control of both houses of the Legislature and the governor's office in the 2010 election just nine months ago.

Democrats had hoped enough wins in the recalls would have allowed them to block the Republican agenda, but the GOP will hold on to their majorities that have allowed them to rapidly pass bills through the Legislature.

The elections were also closely watched in other states undergoing similar partisan battles.

A coalition of unions and labor-friendly groups fighting a Wisconsin-style collective bargaining overhaul in Ohio said the outcome of the recall elections will have little bearing on whether Ohio's law is repealed this fall.

The effort in Wisconsin was about recalling specific Republicans who voted for the anti-union bill while the push in Ohio is about repealing the law itself. That makes it difficult to compare the two states, said We Are Ohio spokeswoman Melissa Fazekas.

Supporters of the Ohio law also are distancing their state from the fight in Wisconsin.

"We're not focused on Wisconsin, and Ohioans aren't looking to another state to tell them where they should stand," said Jason Mauk, spokesman for Building a Better Ohio, a group defending the collective bargaining law.

Ohioans will vote Nov. 8 on whether to accept or reject the union-limiting law signed by Republican Gov. John Kasich in March that limits bargaining rights for more than 350,000 police, firefighters, teachers and other government employees.

Unlike Wisconsin, Ohio's Constitution makes no provision for recalling elected officials.

The last shark hunter in Miami (The Week)

New York ? Mark Quartiano has killed over 100,000 sharks, says Juliet Eilperin, and he's starting to feel a bit uneasy

THE MASSIVE SCALLOPED hammerhead swaying over the deck of Striker-1 brings Rosie O?Donnell to an abrupt halt in the middle of Florida?s Biscayne Bay. Driving a motorboat with a gaggle of female friends, O?Donnell pulls up next to Mark Quartiano?s 50-foot Hatteras and begins peppering him with questions. Pointing to the huge lifeless fish, she asks, ?Is that for real??

Mark ?the Shark? Quartiano, who has operated a fishing charter here in Miami since 1976, is pleased with the attention. He works hard, seven days a week, and it?s strangers? fascination with sharks that keeps his operation humming. The captain assures O?Donnell that the nearly 9-foot fish is genuine. She tells him that her son Blake is a shark aficionado who has spied Striker-1 gliding by their vacation home on Star Island, the exclusive Miami enclave where the pop singer Gloria Estefan and the NBA star Shaquille O?Neal also own manses. ?My son?s going to flip out!? she exclaims, before hurrying back to the island.

Within minutes, O?Donnell returns on a gleaming dark red Jet Ski with Blake in tow. She is unabashed in her admiration of the hulking mass hanging by a rope, its black eyes on opposite ends of its rectangular head now glassy.

?We can?t believe it!? she says, as she and Blake stroke the creature. ?Oh my God. He feels like rubber.?

O?Donnell wants to know who caught the shark, and Quartiano points to Stephanie Perez, a recent Texas Woman?s University graduate who?s about to pursue her master?s degree in speech therapy. It was Perez?s idea to book the charter this morning, bringing along her parents, boyfriend, brother, and brother?s friend for the ride.

This brings another flurry of praise from O?Donnell. ?And you?re the one who caught it,? she marvels. ?Girl power!?

Later on, Perez says she isn?t interested in making a wall mount of her catch: ?Probably pictures are enough for me.? And she remains agnostic about whether what she?s taken part in today is good or bad. ?I know this sounds harsh, but I guess it depends on how many sharks there are,? she says. ?If they?re endangered, you shouldn?t do it, but if they?re bountiful...? Her voice trails off. ?I?m really kind of indifferent on it, because I don?t know enough to say anything.?

Perez will walk off the boat without her big fish?which is, in fact, classified as endangered by the International Union for Conservation of Nature. Regardless, the sight of it is enough to convince O?Donnell that she should take Blake shark fishing aboard Striker-1. As the two of them zoom away on their Jet Ski, Quartiano notes with satisfaction, ?I don?t think she?s Greenpeace, right? That?s a future customer.? The shark has done the captain?s advertising for him. ?People ask me why I hang them up,? he says. ?That?s why I hang them up. If you let them go, you don?t have anything. You almost don?t have a story.?

IF YOU?RE LOOKING for a 21st-century incarnation of Captain Quint, the obsessive shark hunter from Jaws, Quartiano comes pretty close. While he?s a friendlier, more service-oriented version, the Florida charter-boat captain has built his entire professional reputation on his ability to slay the scariest sharks in the sea. Quartiano used to hunt sharks for his own amusement off Miami Beach, but he?s spent most of his career ensuring that other anglers can tell their own big-fish stories. He started out working as a police officer and then became a firefighter, at which point he managed to work four days a week and fish the other three days. Once he cobbled together enough sponsors to support himself by fishing full-time, he made the switch, and at this point he?s the only charter operator who still targets sharks. By his own estimate, he has killed at least 100,000 sharks over the course of his career: As he likes to joke, he?s outlasted his competitors, as well as the scores of sharks he?s hauled on board over the years.

Quartiano models himself in part after legendary shark hunters like Frank Mundus, who fished off Long Island in the 1950s and ?60s?when there were still plenty of sharks around to catch. Mundus earned the nickname Monster Man for the sharks he caught off Montauk, N.Y., and claimed he was the inspiration for Captain Quint, though author Peter Benchley said the character was a composite. Mundus caught two massive great whites in the course of his 40-year career, but eventually he embraced conservation, retired to Hawaii in 1991, and largely gave up shark hunting. Quartiano, however, has yet to temper his pursuit.

In fact, Mark the Shark prides himself on finding new species to kill in order to satisfy his customers, like the thresher sharks he?s managed to cull from a nearby area where they gather to give birth. He is no longer allowed to catch threshers under state law, which complicates this task. While he?s careful to adhere to state and federal rules, he thinks people apply a double standard when it comes to shark fishing. ?You get people who don?t like to hurt animals, but they?re mostly hypocrites,? Quartiano once told a local magazine. ?They want to release everything; meanwhile they go home and eat big juicy steaks.?

Quartiano cannot be called a hypocrite: He lets people know exactly where he stands. He parks his boat right outside one of Miami?s big South Beach hotels, perched next to a black-and-white sandwich-board sign that extols the virtues of ?Mark the Shark?s Monster Fishing Charters.? The tiny beachcomber shop he runs is yards away, its walls lined with at least a hundred shark jaws. ?This is nothing, I?ve got a warehouse full of them,? he explains as I marvel at the dry, jagged teeth looming above me. He even looks the part: Deeply tanned, with a weathered face and blond-streaked hair, he wears sunny yellow fishermen?s overalls that are both practical and symbolic, evoking America?s fishing past.

It is harder work now pleasing his customers than it was in the past, and he blames commercial fishermen who set long-lines. These fishing lines with baited hooks frequently end up snaring sharks, which then drown. There?s no question that the sharks caught unintentionally from such activities, known as bycatch, far outnumber the targeted fish, be it tuna or swordfish.

?Those long-liners do more damage in a night than we do in a year,? he says. And Quartiano simply does not believe that species such as bigeye thresher sharks are endangered, because he still hauls them in on his rod and reel. ?I?ve caught more than anyone else on the planet. There?s no way they?re endangered.?

Data collected by the National Oceanic and Atmospheric Administration?s Fisheries Service tell a different story: Federal officials estimate that recreational landings of large coastal sharks outpaced commercial catches for 15 out of 21 years between 1981 and 2001, with U.S. recreational anglers catching 12 million sharks, skates, and rays in 2004 alone. At this point, NOAA estimates that recreational anglers in the U.S. catch roughly 200,000 sharks a year. Apparently, all those bachelor and bachelorette parties add up.

It?s a classic case of tunnel vision: Humans fail to comprehend the massive impact of our collective activities on the planet because we think of ourselves as lone actors. In 2007, for example, 12 million anglers made nearly 87 million fishing trips on the Atlantic, Pacific, and Gulf coasts, catching roughly 468 million fish. That?s more than one fish for every man, woman, and child in the United States, and then some.

Boris Worm, a marine biologist at Canada?s Dalhousie University, puts it another way: ?Say you have only one in a thousand Americans catch a shark each year. That?s 300,000 sharks a year, just like that. I don?t think we understand how many of us there are.? We dwarf every other large animal on earth in terms of numbers, and that has consequences.

That?s the message carried by marine biologist Neil Hammerschlag, director of the R.J. Dunlap Marine Conservation Program at the University of Miami?s Rosenstiel School of Marine and Atmospheric Science. Hammerschlag relentlessly attacks Mark the Shark and other recreational shark hunters, using every forum he can to question their activities?debating Quartiano in a sportfishing magazine or crusading against him on his website, NeilHammer?.com. The elaborate website features not only a section on how recreational shark fishing is taking its toll (including a photograph of Quartiano displaying two massive dead sharks) but details about conservation efforts and Hammerschlag?s own research.

Hammerschlag has mulled the idea of mobilizing activists to set up a picketing operation near the Marriott to discourage the weekend warriors who patronize Quartiano?s business. In the end, though, he realized it would only give Quartiano additional media exposure and motivation to ?live up to the legend he?s trying to create for himself.?

ANY SHARK THAT swims, I can kill,? Quartiano says, burnishing his legend with little provocation. ?I get paid to kill fish. Some people don?t like it, but too bad.? If environmentalists have such a big problem with his activities, he reasons, they should pay him to park his boat. In an era of government bailouts, Quartiano jokes, he?s happy to take his place in line at the federal trough. ?Basically, my ultimate goal is to get subsidized by the government. They pay farmers not to farm, right? I want to get paid not to fish.?

Despite his bravado, Quartiano has been feeling a little uneasy lately about his activities. He even acknowledges occasionally that he might be contributing to the fishes? demise, though he still sees long-liners as the No. 1 enemy. He wants sharks to stick around long enough for his son Maverick?whom he describes as ?fearless??to kill them. So he?s been doing his part, bringing some of his catches back to local scientists, which he says more than justifies his business.

?How else are you going to get data on some species of sharks?? he asks, adding that it?s no use throwing them back into the sea if they?ve died by the time they?re hauled into the boat. ?This is how they?re going to get them. They may not like it. They?re specimens. Like it or not, we?re going to catch ?em. If we catch an endangered species, why should we let it sink to the bottom?? He also says he tags 200 to 250 sharks a year for NOAA?s Fisheries Service.

But his main business remains helping his customers catch sharks. And they can find his boat parked outside Miami?s big South Beach hotels, as it has been for 35 years. Bachelor and bachelorette parties welcome, all the wine and beer they can drink. If they?ve got $1,200 for a daylong ride, they?ll probably come home with a trophy. He?s the last shark hunter in Miami, a survivor of close calls at sea and a business that pitches up and down along with the economy. In the end, he will hazard only a guess as to whether he thinks his prey will survive over the long term. ?Sharks are cool,? he says, shortly before I head out to catch my water taxi back to shore. ?Hopefully, they?ll be here after we?re gone.?
