Water & Waste - Mexico - Conagua invests over US$30.3mn in waterworks in Baja California Sur

Mexico's national water authority Conagua has invested over 400mn pesos (US$30.3mn) so far this year in Baja California Sur state to improve water supply and flood prevention, Conagua said in a statement.

To improve water supply in the state and as part of a drought mitigation program, Conagua has invested 42mn pesos in the Carrizal-La Paz water pipeline, which includes seven interconnected wells and a 33km distribution pipeline. The pipeline is expected to be completed by 2013, with a total investment of 160mn pesos.

Another 200mn has been invested in La Palma and La Higuerilla dams to retain storm water runoff to prevent floods and increase water availability for human consumption. Works are expected to conclude by 2014 and demand total investment of 484mn for La Palma and 160mn for La Higuerilla.

In addition, Conagua has approved 150mn for the channeling of the Insurgentes stream to reduce flood risks.

Conagua is also investing over 30mn pesos to renew equipment in Los Cabos to obtain more accurate hydrometeorological information.

(AP) ? Romania's unpopular president was fighting for his political life on Sunday as Romanians voted on whether to impeach him, part of a political battle that has raised questions about the rule of law in the fledgling European Union member.

Traian Basescu's rivals in the government are seeking to impeach him for the second time in five years. They claim the 60-year-old populist violated the constitution by meddling in government business, coddling cronies and using the secret services against enemies.

Basescu, a former ship captain whose popularity has plummeted over economic challenges, says the impeachment process is a political vendetta carried out by opponents and has urged his supporters to boycott the vote ? a tactic that may help him survive thanks to a rule requiring turnout to be more than half of the total electorate.

The political turmoil has dented Romania's credibility, with the U.S. and EU expressing doubts about the left-leaning government's respect for the independence of the judiciary. Critics accuse Prime Minister Victor Ponta, himself the subject of a plagiarism scandal, of orchestrating the move as part of a power grab.

Polls opened at 7 a.m. (0400 GMT) and will close at 11 p.m. (2000 GMT) with 18 million Romanians eligible to vote. Most are expected to vote to impeach Basescu, but it is uncertain whether the government can muster the necessary turnout.

Basescu, who has been president since 2004, saw his approval numbers drop after government introduced austerity measures in 2010 to meet demands by the International Monetary Fund in exchange for a multibillion euro (dollar) loan. The government cut public wages by one-fourth and raised sales tax to 24 percent, one of the highest in the EU.

Ponta says that Basescu's confrontational style of governing and interfering in the justice system and government business are evidence that he is unfit for the job. He says that charges that he plagiarized his 2004 doctoral thesis are orchestrated by Basescu's camp.

The foreign ministry has opened polling stations in embassies in the United States, Italy, Spain, France and elsewhere where an estimated 2 million Romanians are eligible to vote.

The patient-doctor communication/decision process in Europe ...

An interesting but underpowered study just reported as a ?mini-review? in BJU International gives us some (but not much) new information from Europe about the comparative expectations of urologists and prostate cancer patients around the discussion of new diagnosis and the the choice between appropriate treatment options.

The sad thing about this paper is that while the authors (Dennis et al.) generated information from 303 physicians, they got input from 48 patients ? most of them in Belgium and all diagnosed at least 5 years previously.

Those who subscribe to the UroToday web site will be able to read the full text of the paper. Others will be limited to accessing only the abstract (which, also sadly,?doesn?t mention the levels of participation by physicians or patients in this study).

The study was based on two linked surveys carried out in 2011. One was focused primarily on?urologists (291/303 physicians)?who treat prostate cancer, about half of whom?practised at academic medical centers. The other was directed to?patients with prostate cancer but limited (perhaps not intentionally) to men diagnosed at least 5 years earlier, which seems like a serious lack of commitment to gaining good, current patient input. Results from the two surveys?are?complemented with published data from other sources.

According to the published results:

  • Most specialists spent 15-29 min telling the patient about his?diagnosis.
    • About 50 percent of the specialists felt ?very comfortable? during this conversation.
    • About 67 percent of the patients reported feeling uncomfortable
    • About 40 percent of the patients stated that their physician was insufficiently supportive or sensitive
    • 65 percent of the patients considered this time insufficient.
  • Most specialists spent?another 15-29 min?explaining treatment options.
    • 52 percent of?patients considered this to be insufficient.
    • Physician and patient opinions about the prognostic and therapeutic information provided were very different.
    • ?Nearly all? physicians reported providing patients with information about?effectiveness/prognosis and side effects of different treatments
    • Only 56.3 percent of patients remembered being given information about effectiveness/prognosis
    • Only 29.2 percent?of patients remembered?being given information about side effects.
    • About 67 percent of patients were dissatisfied with the amount of information provided.
  • Only 10 percent of patients were provided with any printed information that they could read and absorb in a less stressful situation.
  • A shared decision-making process was (supposedly) preferred by both patients and specialists.
  • Both the physicians and the patients considered treatment efficacy to be the key?factor determining treatment choice. However, ?
  • Physician opinion and/or experience?had a?significant impact on patient choice regarding treatment.?
  • The supportive role of partners/relatives was more appreciated when discussing treatment options than during diagnosis.

In their conclusions the authors state that physicians can (and arguably should) seek to?improve quality of care by

  • Taking adequate time?to inform and educate?their patients
  • Using simple terminology and straightforward language that is easily understood by patients
  • Encouraging a shared decision-making process.
  • Ensuring that?the individual patient?s needs and preferences?are the clinical priority

While these are all laudable conclusions, the lack of large-scale patient input into the databases on which the report is?built demonstrates that the research team itself completely failed to acknowledge the importance of gathering such input with accuracy from a large and well-characterized cohort. (Within the report, for example, they describe a separate but similar patient survey with data from 75 patients?as ?extensive.?) The authors do, however, acknowledge that the high proportion of academic urologists in the physician sample may also mean that the physician input was not entirely reflective of widespread community practice.

The ?New? Prostate Cancer InfoLink is of the opinion that research into the communication process between physicians and patients is still not being taken seriously by most funding bodies. This report, which?includes the leader of a European?advocacy organization (a retired urologist) as the primary author,?appears to confirm our opinion. It is to be hoped that the Patient-Centered Outcomes Research Institute (PCORI) here in the USA will be willing to fund much better research in this area and encourage a greater appreciation of the importance of this issue.

Charitable giving grows in Charlotte

Contributions, gifts and grants to private foundations in the 14-county Charlotte region grew to $170 per capita in 2010 from $71 per capita in 2009, while growing to $602 from $574 per capita for public charities in the same period, according to the Urban Institute at the University of North Carolina at Charlotte.

Still, while contributions to public charities in the region seems to be on a steady rise, giving to private foundations has been much more sporadic over the years, the Urban Institute at UNC-Charlotte says, citing two annual reports published by the National Center for Charitable Statistics at the Urban Institute in Washington, D.C.

Contributions to private foundations grew between 2009 and 2010 in seven counties in the region, including an increase to $434 per capita from $176 in Mecklenburg County, which posted the biggest increase in the region, and a decline to $5 per capita from $15 in Cleveland County, which posted the biggest decline.

Per-capita giving to public charities grew in 10 counties in the region in that two-year period, including an increase to $948 per capita in Lancaster County, S.C., the biggest increase in the region, and falling to $308 per capita from $500 in Cabarrus County,? which posted the biggest decline.

Back when Sony announced the Xperia GX and Xperia SX there was no mention of any launch outside of the company's native Japan. The manufacturer was quick to highlight the fact that these devices designed specifically for the Japanese market and that they had no news to share at the time about any international launch plans. That said though, the Xperia GX  has now popped up at the FCC and is noted as having a pentaband 3G radio onboard to go along with Android 4.0, a 4.6-inch 720p Sony Reality display and a 1.5GHz dual-core CPU. In other words, a Sony device a lot of folks might actually be interested in if it was made readily available outside of Japan.

Java developer most difficult of IT jobs to fill - ComputerworldUK.com

Java developers remain the most difficult tech pros to land, followed by mobile developers, .NET developers and software developers, according to a recent survey.

Hiring managers and recruiters cite these positions two- or three-times more frequently than other skill sets in the employment marketplace, according to Alice Hill, managing director at Dice.com, which conducted the study in the US.

The IT jobs site polled 866 tech-focused hiring managers and recruiters to come up with its list of hard-to-fill positions. Rounding out the top 10 list are candidates with skills related to: security, SAP, SharePoint, web development, active federal security clearance, and network engineering.

In general, companies are looking for candidates with at least a few years of experience.

"Asked for experience preference, corporate hiring managers most frequently say IT pros with two to five years in the workforce, followed by those with six to 10 years of experience," Hill said. "Competition is fierce when companies are all chasing the same talent, making positions hard to fill."

Another reason for tight competition on the hiring front is that companies are less focused on internal talent development than they have been in the past, and formal corporate IT training opportunities aren't widespread.

"Companies have been shifting the responsibility for training their employees to the individual for decades. Hiring managers say they expect tech professionals to stay with their firm about three years. That makes it tough to cross-train, retrain, or train at all," Hill says.

As of early July, Dice lists 84,940 available tech jobs in the US. Geographically, the New York/New Jersey region came out on top (8,871 jobs) when Dice ranked the top tech metro areas by number of open job postings. Washington, DC/Baltimore is ranked second, with 8,334 jobs, followed by Silicon Valley with 5,684 open positions.

Compared to a year ago, the region with the largest job gain is Los Angeles. Dice lists 3,551 available jobs in the LA area, a gain of 14% compared to July 2011.

About 3,300 people are diagnosed every year with CLL, the most common form of leukemia in Western countries. Researchers have known for quite some time that certain families are more susceptible to sustaining CLL than others. However, the genetic basis for inherited predisposition to CLL has so far been unknown.

Researchers from London?s Institute of Cancer Research (ICR) in the UK have now identified a heritable gene variant that is linked to a higher risk of developing chronic lymphocytic leukemia (CLL). The study, published in the journal Blood, reports the researchers discovery of another key region of the genome that is associated with the inheritance of CLL.

The ICR team?s recent studies have demonstrated that there is no single ?cancer gene responsible for CLL, but that susceptibility is potentially due to the co-inheritance of multiple risk variants that elevate the risk when inherited in combination. The researchers hope that better insight into the inheritable genetic factors that drive the development of cancer can help in designing new drugs and screening methods to target those who are at high-risk.

In a meta-analysis that consisted of combined data from three independent genome wide association studies (GAWAS), the researchers analyzed genetic data from 1,121 leukemia patients and 3,745 healthy controls and found a new CLL ?risk? inherited variant, which was situated at a very specific point on a single chromosome encoded as 6p21.33.

Leading researcher, Professor Richard Houlston, of the molecular and population genetics team at The Institute of Cancer Research, explained:

?We investigated the combined effect of this newly found risk variation and other known CLL related variants. There was no evidence of interaction between the genes, indicating that each locus plays an independent role in increasing risk.?These gene variations contribute significantly to the development of CLL. While risks associated with each variant are individually modest, these variations are frequently carried by individuals of European ancestry.

This variant is the thirteenth that we have discovered ? so while each one alone has a small effect on the risk of developing CLL, in combination these risks are important. The genetic variants we have found so far account for around 16 per cent of the inherited risk of CLL, so we believe there are more genetic variants still to be found.?

The researchers found a strong link between the ?risk? locus at the 6p21.33 location and lower expression of the BAK1 gene, which has a vital impact in maintaining levels of B cells, i.e. healthy immune cells in the blood. BAK1 deficiency was noted to prevent natural cell death, which leads to an accumulation of immature and mature B cells. The discovery provides new insight into the biological development of CLL.

Professor Chris Bunce, Research Director of Leukemia Lymphoma Research, concluded:

?This study offers more compelling evidence for the role of inheritable risk genes in hijacking the natural cell cycle.? More research is needed to establish just how significant this newly discovered risk gene variant is in the development of CLL.?
